Make their day…and yours!
As hostess, you can invite eight people to get their heart health assessments with A Change of Heart. We'll send each of them a packet of information to help them prepare for their assessment and your group’s special day together.

Your day will begin in our heart center at 8 a.m.  You can greet your guests and congratulate them for taking care of their hearts. For the next four hours, each of your friends will receive an individual assessment, including a one-on-one results review with a cardiac nurse practitioner. You’ll have plenty of time to visit with your friends – and with our staff, who will help each of your guests learn how to manage their risk factors and live heart-healthy lives.  A heart healthy breakfast will be available for your guests and you to enjoy.

At $60 per person, the total fee for the day and all eight assessments is $480. It's truly a gift from the heart.

As part of their heart health assessment, your guests will receive:
